Jefferson Health Plans in Philadelphia is seeking a Lead Community Health Worker to guide a team of CHWs, connect community members with health services, and address social determinants of health across Jefferson sites in the Philadelphia region. A PA CHW certification is required, and some evening/weekend hours may be expected.

The role involves traveling to multiple locations, home visits when needed, and ongoing collaboration with community organizations to promote health equity and strong

Competenze

• Active Community Health Worker certification for PA required.
• Experience coaching/mentoring CHWs and developing training.
• Ability to travel to multiple locations within a day.
• Willingness to conduct home visits (may not apply to all locations).
• Excellent communication and de-escalation skills.

Mansioni

• Lead and mentor CHWs, coaching on navigation and healthy behaviors.
• Serve as SME on Health Related Social Needs assessments and EHR workflows.
• Collaborate with managers to develop trainings, curricula, and CE content.
• Coordinate oversight and support for community events.
• Manage a modified client caseload and ensure timely documentation.
• Apply DEI principles in interactions with clients and staff.
