Job summary

Privia Health seeks a Population Health Quality Coordinator to execute defined value-based program metrics and coordinate care for at-risk patients. You will engage in patient outreach, gap closure, and action on performance reports and regional plans.

The role requires clinical or licensure background, phone-based communication, EHR familiarity, and HIPAA compliance. Collaborative work with PCPs, care centers, and partners is essential.

Qualifications

• MA, EMT, Case Management certification and/or clinical experience preferred
• Preferred experience with profession-based telephonic or remote communication
• EHR experience preferred
• Must comply with all HIPAA rules and regulations

Responsibilities

• Conduct patient outreach to support quality-related programs and ensure target goals are achieved
• Assist in scheduling PCP and specialist patient appointments
• Prepare patient, provider, and care center level reports from the EMR and payer portals
• Review performance data for assigned region and create action plans that support organizational goals
• Build collaborative working relationships with care centers and population health specialists
• Demonstrate critical thinking to determine the best way to support care centers
• Answer and resolve business office inbound calls related to population health or other clinical activities
• Make referrals to internal care team, payer programs, community resources, home care, and disease management programs
