Job summary

Southeast Primary Care Partners is seeking a Population Health Transformation Specialist to drive the shift to outcomes-based care across multiple clinics. You will lead site visits, support providers in using population health tools, and develop pathways to value-based care with a focus on RAF, quality measures, and closing care gaps.

You will train staff, monitor program performance, and maintain alignment with HEDIS and STARs while collaborating with practice leadership to achieve population

Qualifications

• Experience with HEDIS, STARS ratings and population health initiatives.
• Proficient use and documentation in EHRs; Allscripts/Veradigm preferred.
• Experience training staff on value-based programs and requirements.
• Auditing documents for gap-closure and quality reporting.
• High-school graduate; an Associate degree is a plus.

Responsibilities

• Lead transition to value-based care across clinics and partners.
• Support providers and staff with population health tools and pathways.
• Develop and implement care pathways addressing RAF reporting and quality measures.
• Track performance goals and deliver ongoing training on population health initiatives.
• Review data and communicate improvements to leadership; ensure regulatory alignment.
