The role requires strong leadership, collaboration across regions, and expertise in clinical documentation, analytics, and program design to drive measurable improvements in population health outcomes.

Qualifications

• 5+ years of healthcare/population health experience
• Excellent communication skills
• Ability to build trust with a wide range of stakeholders and hold teams accountable
• Demonstrated history of strong leadership and data-driven decision making
• Graduate degree preferred (MBA, MPH)
• Bachelor's degree required
• US work authorization

Responsibilities

• Own strategic planning, execution and refinement of Population Health programs in the respective region
• Work with regional leaders and center care teams to support the Oak Street care model and documentation excellence, workflow optimization, and avoidable admission prevention
• Develop scalable solutions for accurate clinical documentation and avoidable admission prevention across centers
• Collaborate with central office to design strategy and program for documentation and medical cost/ admission efforts
• Analyze data to monitor performance, identify opportunities, and drive regional population health execution
• Recruit, retain and engage medical scribe cohort in the region
• Be accountable to key population health metrics (quality, utilization, financial)
• Other duties as assigned
