Responsibilities

• Support execution and ongoing refinement of Population Health programs.
• Collaborate with Product, Finance and Data Science to design programs.
• Work with regional leaders to implement clinical and business priorities.
• Lead communication and content development for population health metrics.
• Other duties as assigned.
