Job summary

Oak Street Health is seeking a Population Health Director in the St. Louis area to lead local Population Health programs and partner with care teams, providers, and operations.

The role focuses on transitions-in-care, medication adherence, risk adjustment, and patient engagement, with an emphasis on measurable quality and cost outcomes. You will report to the Regional Vice President and work to accelerate improvement across Oak Street Health divisions.

Qualifications

• 5+ years in healthcare/population health or related field.
• Bachelor's degree required; MBA/MPH preferred.
• Ability to build trust with diverse stakeholders.
• Analytical and data-savvy; ROI/financial analysis skills.

Responsibilities

• Own the execution and ongoing refinement of Population Health programs in the local division.
• Collaborate with central office leaders to design strategy and programs.
• Lead cross-functional teams with providers, care teams, and operations.
• Track quality, utilization, and financial metrics to drive outcomes.
• Perform other duties as assigned.
