The director will serve as a key subject matter expert and cross-functional partner, helping shape initiatives that strengthen care coordination and support market-wide goals.

Qualifications

• Bachelor's degree in nursing, social work, health services research, health policy, information technology, or a related field.
• Minimum five years of experience in population health, service coordination, or related fields.
• Experience working in Medicaid.
• Proficiency in Microsoft Word, Advanced Excel, and PowerPoint.
• Ability to analyze data for quality improvement recommendations.
• Excellent interpersonal skills for relationship building with diverse stakeholders.
• Experience in program planning, implementation, and evaluation.

For our client, we are seeking a Population Health Director to join the team of a leader in the Healthcare space. This role will lead population health strategy and operational planning for a remote healthcare environment, with a strong focus on identifying patient needs and improving outcomes across diverse populations. The director will serve as a key subject matter expert and cross-functional partner, helping shape initiatives that strengthen care coordination and support market-wide goals. This position requires a blend of strategic thinking, data-informed decision-making, and stakeholder collaboration to advance quality improvement efforts.

Location: Remote - US based candidates only, no visa sponsorship available

Compensation: $150,000 – $206,300 annually
