Job summary

Ladders is seeking a Population Health Director to lead population health strategy and operational planning for a remote healthcare environment, with a strong focus on identifying patient needs and improving outcomes across diverse populations.

The director will serve as a key subject matter expert and cross-functional partner, helping shape initiatives that strengthen care coordination and support market-wide goals.

Qualifications

• Bachelor's degree in nursing, social work, health services research, health policy, information technology, or a related field.
• Minimum five years of experience in population health, service coordination, or related fields.
• Experience working in Medicaid.
• Proficiency in Microsoft Word, Advanced Excel, and PowerPoint.
• Ability to analyze data for quality improvement recommendations.
• Excellent interpersonal skills for relationship building with diverse stakeholders.
• Experience in program planning, implementation, and evaluation.

Responsibilities

• Identify health needs, including chronic diseases and disabilities, to inform departmental strategy
• Develop strategic designs and operational plans for population health initiatives
• Serve as a subject matter expert and liaison for population health activities
• Provide insights to improve care coordination and overall health outcomes
• Collaborate with cross-functional teams to support market-wide initiatives
