Job summary

United Health Centers of the San Joaquin Valley is seeking a Population Health Specialist to provide clerical and data support for value-based care initiatives in Fresno, CA. The role supports the Population Health Director, helps design dynamic presentations, tracks improvement initiatives, and coordinates meetings.

The ideal candidate has 1+ year clerical healthcare experience, familiarity with HEDIS measures and RAF scores, and proficient in Word/Excel/PowerPoint.

Qualifications

• AS/AS or equivalent required; Bachelor's preferred.
• Healthcare-related degree or certification preferred.
• 1+ year clerical or healthcare experience; value-based experience beneficial.
• Experience with HEDIS or value-based healthcare measures.
• Familiarity with codesets and RAF scores.
• Advanced Word/Excel/PowerPoint experience.

Responsibilities

• Support the Population Health Director with clerical and data tasks.
• Assist with value-based quality improvement projects and reporting.
• Develop and present results to stakeholders; create presentations.
• Coordinate meetings, records, and tracking of improvement initiatives.
• Identify gaps in care and support follow-up with specialists or providers.
• Collaborate with departments to improve quality metrics and ROI.
• Maintain confidentiality and adhere to HIPAA and organizational policies.
• Train UHC staff on value-based initiatives and quality measures.
