Job summary

United Health Centers is seeking a Population Health Specialist to provide clerical and data support for value-based care initiatives. The role assists with both clerical and analytical functions of all value-based quality improvement projects, and communicates outcomes while designing dynamic presentation materials.

Qualifications

• AS/AS or equivalent required.
• Bachelor’s Degree preferred.
• Degree/certification in healthcare related field preferred.

Responsibilities

• Support the Population Health Director to meet clinical standards of care and HEDIS/STARS goals.
• Gather information, analyze context, and draw conclusions for improvement.
• Create and develop visual presentations for quality improvement activities.
• Coordinate tracking and reporting of clinical outcomes and corrective action plans with IPAs and Health Plans.
• Educate and train UHC staff on value-based initiatives and quality measures.
• Maintain confidentiality and HIPAA compliance in all tasks.
