Job summary

Allegheny Health Network is seeking a Medical Assistant to support population health management, coordinate care, and assist providers during visits. You will engage with patients, help manage chronic conditions, and contribute to improving care delivery in a team-based setting.

Responsibilities include identifying gaps in care using registries, performing pre-visit planning, assisting with rooming and order entry, coaching patients through self-management, and supporting data quality

Qualifications

• 2 years of Medical Assistant experience.
• CPR certification.
• Act 34 Criminal Background Clearance Certificate
• Act 33 Child Abuse Clearance Certificate
• Act 73 FBI Fingerprinting Criminal Background Clearance Certificate

Responsibilities

• Identify patients with gaps in care using analytic tools and registries, and help close gaps.
• Complete pre-visit planning by reviewing charts and preparing for patient visits.
• Assist with rooming and activating standing orders to address care gaps.
• Document elements of the clinical visit under supervision and assist with order entry.
• Use motivational interviewing to coach patients toward self-management and activation.
• Review population health data and perform outreach to close care gaps.
• Identify high-risk patients and coordinate complex care referrals with care team nurses.
• Collaborate to improve access to care and reduce avoidable ER visits.
• Clean and sterilize instruments and manage inventory of medical supplies.
• Perform in-house testing following established standards and policies.
• Perform other duties as assigned.
