Job summary

North Arkansas Regional Medical Center is seeking a Population Health Specialist to support preventive care, chronic disease management, and care coordination. You will work with patients and care teams to identify gaps in care and coordinate outreach to ensure timely services.

The role involves reviewing clinical data, supporting PCMH and CPC+ initiatives, and connecting patients with resources while promoting continuity of care.

Qualifications

• High school diploma or equivalent is required.
• Current CCMA, RMA, or CMA certification, or an active LPN license is required within six months of employment.
• Experience in patient outreach, care coordination, medical records, or preventive healthcare initiatives is preferred.

Responsibilities

• Review patient data to identify gaps in preventive screenings and chronic disease management.
• Coordinate patient outreach for preventive care and follow-up appointments.
• Contact patients for pre-visit planning and follow-up care, including high-risk patients.
• Verify the accuracy of patient records and program participation data.
• Identify barriers to accessing healthcare and connect patients with resources.
• Provide basic health education and support informed health decisions.
• Monitor access to services and linkage to the medical home.
