• Prioritize patients for program outreach using reports, risk stratification, and referrals.
• Coordinate education, referrals to and from providers, and community programs.
• Track patient data, stratify registries, and generate requested reports.
• Support pre-visit information and reminders for labs, tests, and appointments.
• Assist with team huddles, meetings, and scheduling.
• Ensure HIPAA compliance and protect patient confidentiality.
