Job summary

AllCare Health in Brookings, Oregon, is seeking an Intensive Care Coordinator to identify health-related social needs, educate members on benefits, and coordinate care with the multidisciplinary team. This role involves navigating Medicaid/Medicare networks and maintaining detailed care plans across settings.

The position requires on-call duties, travel as needed, and strong organizational and communication skills within a fast-paced Population Health team.

Qualifications

• This position requires a high school diploma or GED and relevant health care exposure.
• Experience with Oregon Health Plan/Medicare rules is preferred.
• Certification in Mental Health First Aid or willingness to obtain within 90 days.

Responsibilities

• Identify social determinants of health needs for members.
• Navigate community services and educate members on benefits.
• Coordinate care plans and document in Care Management System.
• Act as liaison between members, ICT, providers, and DHS staff.
• Maintain attendance, participate in trainings, and meet deadlines.
