Job summary

INTEGRIS Health in Oklahoma City seeks a full-time RN Care Coordinator for Population Health Systems of Care. The role focuses on coordinating clinical and social aspects to improve health outcomes, with emphasis on transitions, education, and collaboration across care teams.

You will work in a supportive environment with access to INTEGRIS’ extensive network, front-loaded PTO and opportunities for continued education.

Qualifications

• RN license in Oklahoma or a multistate license under eNLC.
• Bachelor's degree in Nursing preferred.
• Minimum of 3 years of clinical experience in diverse settings.
• BLS certification within 90 days of hire and renewals as required.
• Care Management/Coordination experience preferred.
• Strong interpersonal and collaboration skills.
• Computer experience; Windows and Excel preferred.
• Valid Oklahoma driver’s license with acceptable driving record.

Responsibilities

• Assess patients across clinical, psychosocial, financial and other factors affecting health outcomes.
• Evaluate social determinants that may elevate risk.
• Collaborate with care team to deliver high-quality, patient-centered care.
• Provide patient education and engage patients/families in goal setting.
• Coordinate with community resources and partners to support the plan of care.
• Utilize shared decision making to develop clinically appropriate plans aligned with patient choices.
• Identify barriers to achieving goals and develop strategies to reduce readmission risk.
• Document and ensure continuity of care across the care continuum.
