Job summary

AllCare Health in Brookings, OR is seeking an Intensive Care Coordinator to identify health-related social needs for Medicaid/Medicare members and navigate community services. You will educate members on benefits, coordinate with care teams, and help develop self-management plans across settings.

The role requires strong organization, communication, and knowledge of HIPAA. Travel and on-call duties are involved, with competitive wages and benefits.

Qualifications

• High school diploma or GED required.
• Six months to one year related experience.
• Valid Oregon Driver’s License and auto insurance.
• Mental Health First Aid Certification or willing within 90 days.
• Traditional Health Worker certification preferred.

Responsibilities

• Coordinate care to meet member health care needs and ensure timely access to services.
• Act as liaison between members, ICT, providers, and DHS caseworkers.
• Maintain Care Management System documentation and prepare care plans.
