Duke Connected Care is seeking a Team Lead to coordinate clinical and functional care management for assigned populations in Durham. You will supervise staff, ensure quality care transitions, and collaborate with PHMO leadership to align care strategies with program requirements.

Responsibilities include staff supervision, performance monitoring, and participating in staffing decisions while maintaining a small caseload to support care planning and documentation timelines.

Qualifikationen

• Bachelor’s degree in clinical field (Nursing, Counseling, Social Work, Therapy, Allied Health, or related).
• 3 years of clinical experience required.
• Extensive management and care/case management experience required.

Aufgaben

• Coordinate timely implementation of interventions for identified populations following policies.
• Monitor staff performance and engage management to improve care and outcomes.
• Participate in hiring/firing processes of care management staff.
• Conduct monthly supervision and team meetings with direct reports.
• Shadow direct reports during evaluation periods and provide training on workflow and documentation.
