Duke Connected Care seeks a Population Health Resource Associate in Durham to implement and coordinate comprehensive patient plans, enabling recovery and management of complex conditions in collaboration with the Complex Care Team(s).

The role involves outreach, education, care coordination, data collection and reporting, and participation in provider meetings to promote coordinated patient care and program effectiveness.

Formación

• High school diploma or equivalent.
• Two years of customer interactions preferred.
• Active user of electronic medical records (EMR) software.
• Ability to organize, document and communicate clearly.

Responsabilidades

• Provide outreach and care management support to patients and coordinate with care teams.
• Coordinate and/or participate in community health activities and education materials.
• Assist with data collection, reporting and program analyses to track progress and trends.
• Lead and supervise new hires and maintain program standards as part of the team.
