Job summary

CommunityCare, Inc. is seeking a Population Health Clinical Assistant to provide administrative and care coordination support to PHM nurses, the clinical pharmacist, social worker, and nurse case managers.

The role organizes referrals, schedules member assessments, coordinates communications, and tracks follow-up tasks so clinical staff can focus on member care. The position involves handling referrals, scheduling, documentation, and assisting with audits and reports while ensuring

Qualifications

• High school diploma or equivalent required.
• Educational background in healthcare or a related field preferred.
• One year of experience in healthcare administration, member services, medical office support, or related setting preferred.
• Experience supporting care management, referrals, scheduling, or health plan operations preferred.

Responsibilities

• Receive, enter, and route referrals to PHM team members.
• Contact members to schedule assessments, appointments, and follow-up calls.
• Organize work queues, track tasks, and notify staff of outstanding needs.
• Request medical records, discharge summaries, and other information from providers.
• Assist with coordinating provider appointments, transportation, and community resources.
• Schedule interdisciplinary care meetings and distribute care plans.
• Support referrals by gathering information and tracking follow-up.
• Prepare and send member and provider correspondence and educational materials.
• Maintain accurate documentation and assist with reports, outreach tracking, and audits.
• Refer clinical questions promptly to licensed staff.
• Perform other duties as required.
