Qualifications

Responsibilities

• Identify residents before care using wristbands or photo cards.
• Report changes in resident condition to Nurse Supervisor.
• Record resident care and observations accurately.
• Maintain confidentiality of resident information.
• Assist with daily personal care (bathing, grooming, dressing, toileting).
• Assist with mobility and safe transfers.
• Provide restorative and rehabilitative care as instructed.
• Observe signs of pressure injuries or skin breakdowns.
• Assist with meal service and document intake.
• Follow infection control and safety procedures.
• Support residents emotionally and create a warm environment.
• Uphold residents' rights and report refusals appropriately.
