Job summary

Responsibilities

• Identify residents before care or treatments using wristbands or photo cards.
• Report changes in resident condition, incidents, or accidents to Nurse Supervisor promptly.
• Record resident care and observations on charts and flow sheets.
• Maintain confidentiality of resident care information, including HIPAA.
• Assist residents with daily personal care needs (bathing, grooming, dressing, etc.).
• Ensure call lights are answered promptly and assist with mobility and transfers.
• Provide restorative and rehabilitative care as instructed.
• Maintain clean, comfortable, and dignified resident rooms.
