Job summary

Comprehensive Rehab Consultants seeks a Nurse Practitioner/PA to deliver PM&R/Physiatry services at designated SNFs. You will lead clinical care, collaborate with interdisciplinary teams, and support patient independence in post-acute settings.

The role emphasizes autonomy, ongoing education, and a strong clinical leadership presence within partner facilities, with a typical 8-hour day and regular Monday–Friday schedule.

Qualifications

• Must be a Nurse Practitioner/APRN or Physician Assistant with Texas license.
• Post-acute/post-acute care experience preferred.
• Confident in autonomous practice and leadership within an interdisciplinary team.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with clinical leadership.
• Collaborate with PCPs and SNF teams to manage common post-acute conditions.
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
