Job summary

Comprehensive Rehab Consultants is hiring a PM&R Clinician (NP/PA) to provide expert Physiatry services at designated SNF/post-acute centers in Pleasanton, CA. Full-time position with a 8:00–17:00 schedule, option for flexible on-site workflows and strong clinical leadership support.

You will partner with PCPs and therapy teams to manage common geriatric and post-acute conditions, advance patient independence, and promote quality outcomes across the facility network.

Qualifications

• Experience in post-acute healthcare settings preferred.
• Ability to work autonomously within SNF/post-acute facilities.
• Strong interpersonal and compassionate patient care approach.

Responsibilities

• Provide PM&R/physiatry services as a rehab consultant in SNF settings.
• Collaborate with interdisciplinary teams and facility leadership.
• Support management of common SNF conditions with rehab focus.
• Coordinate with PT/OT/SLP to improve patient function and outcomes.
• Participate in multidisciplinary meetings and staff education.
