Qualifications

• Post-acute care experience preferred.
• Autonomy in clinical practice.
• Strong interpersonal and compassionate patient care.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in facility through clinical leadership and collaboration with the care team.
• Collaborate with PCP teams on SNF/post-acute conditions.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Track patient progress and address barriers to therapy.
• Attend multidisciplinary meetings and support culture building.
