Job summary
Comprehensive Rehab Consultants is seeking a dedicated PM&R clinician to deliver specialized Physiatry services at skilled nursing facilities in Brewster, Centerville, or Sandwich, MA. This full-time W-2 role offers autonomy within a supportive team and a strong focus on patient independence.
You will collaborate with inter-disciplinary teams to optimize rehabilitative outcomes, participate in ongoing clinician education, and enjoy a competitive salary with education reimbursement and growth
Qualifications
- Nurse Practitioner/APRN/PA with MA license.
- Post-acute/SNF experience preferred.
- Ability to work in the U.S.
Responsibilities
- Provide PM&R/Physiatry services as a rehab consultant.
- Represent CRC at the facility with clinical leadership and care-team collaboration.
- Collaborate with PCPs to manage common SNF conditions (CVA, neurological conditions, postoperative ortho).
- Work with PT/OT/SLP teams to improve function and quality of life.
- Follow therapy patients to track progress and address barriers to therapy.
- Attend multidisciplinary meetings and foster culture-building and education.
- Engage in continuing education including monthly didactics and regional events.
