Job summary

Comprehensive Rehab Consultants in Sanford, NC seeks a Part-time NP/PA specializing in Physical Medicine and Rehabilitation to serve SNF patients. You will deliver PM&R services within designated facilities, collaborating with in-house teams while maintaining autonomy and a focus on patient-centered care.

Qualifications

• Must hold Nurse Practitioner/APRN/PA license.
• Must have North Carolina medical license.
• U.S. work authorization.
• Post-acute/SNF experience preferred.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team.
• Collaborate with PCP teams to help with management of conditions common in SNFs.
• Work alongside your PT, OT, and SLP teams to improve patient function and quality of life.
• Follow therapy patients to track their progress in therapy.
• Assess and collaborate on the management of barriers to therapy including pain and neurogenic issues.
• Attend multidisciplinary meetings and contribute to culture building and education.
• Engage in Didactic lectures and regional team meetings to stay aligned with CRC standards.
