Job summary

Comprehensive Rehab Consultants in Marion, MA is seeking a NP/APRN/PA PM&R Clinician to join our post-acute care team at a designated SNF.

You will collaborate with PT/OT/SLP and PCPs to optimize rehabilitative outcomes, support patient independence, and lead clinical discussions across the interdisciplinary care team. The role is a 32-hour, W-2 position with a 4-day workweek and hours around 9 AM–4 PM.

Qualifications

• NP/APRN/PA license and Massachusetts license required.
• Ability to work in the U.S. and post-acute/SNF experience preferred.
• Clinicians should be autonomous with strong patient-care focus.
• Experience in post-acute care and geriatrics is valued.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNF/post-acute setting.
• Collaborate with the care team including PCPs, PT/OT/SLP to manage conditions.
• Assist with patient documentation and care coordination.
• Lead clinical activities and participate in multidisciplinary meetings.
• Promote patient independence and optimize rehabilitative outcomes.
