Job summary
Comprehensive Rehab Consultants (CRC) seeks a PM&R Clinician (NP/PA) to treat patients exclusively at a designated SNF or post-acute rehab center in Johnston, RI. This full-time W-2 role emphasizes high-quality care, collaborative teamwork, and clinical leadership within the facility.

Qualifications
- Must hold NP/APRN/PA license and RI license.
- Experience in post-acute care settings preferred.
- Strong interpersonal and empathetic patient care approach.
- Ability to work autonomously within SNF/post-acute teams.
Responsibilities
- Provide PM&R/Physiatry services as a rehab consultant.
- Represent CRC with strong clinical leadership in the facility.
- Collaborate with PCPs to manage SNF conditions.
- Coordinate with PT/OT/SLP to improve function and quality of life.
- Monitor patient progress and therapy outcomes.
- Address barriers to therapy and assist in problem solving.
- Attend multidisciplinary meetings and promote culture building.
