Job summary
Comprehensive Rehab Consultants seeks a Nurse Practitioner/APRN to provide PM&R services at designated SNFs and post-acute rehab centers. You will collaborate with an interdisciplinary team to optimize rehabilitative outcomes for geriatric patients in Allentown and Bethlehem, PA.
The role offers a full-time, 40-hour W-2 schedule with a typical 8 AM–5 PM day, competitive salary, and opportunities for CEUs and career growth within CRC. Must hold PA license and be able to work in the U.S.
Qualifications
- Post-acute healthcare experience is essential.
- Ability to work autonomously within SNF/post-acute settings.
- Strong interpersonal and collaborative skills with interdisciplinary teams.
- Commitment to patient-centered rehabilitation and quality care.
Responsibilities
- Provide PM&R/Physiatry services as a rehab consultant.
- Represent CRC in your facility with clinical leadership.
- Collaborate with PCP teams for SNF/post-acute conditions.
- Work with PT/OT/SLP to improve function and QoL.
- Track patient progress and therapy outcomes.
- Address barriers to therapy including pain and neuro conditions.
- Attend multidisciplinary meetings and support culture building.
