Job summary

Kansas Health System is seeking an Ambulatory Clinic RN Care Coordinator for the Plastic Surgery team. This full-time role focuses on safe, evidence-based nursing care, patient advocacy, and coordinating healthcare services within clinic sessions.

The RN will support physicians, educate patients, manage high-risk cases, and ensure plans of care are understood by patients and families, while maintaining patient confidentiality and accurate EMR documentation.

Qualifications

• High School Graduate.
• Bachelor Degree in Nursing.
• 2+ years of RN experience.

Responsibilities

• Manage patient flow, triage, and direct patient care.
• Liaison between front desk staff, nursing staff, physicians, and clinic manager.
• Develop expected patient outcomes and coordinate care with providers.
• Provide nursing support and education to improve patient outcomes and quality of life.
• Coordinate continuity of care after hospital admission, discharge, and ER visits.
