Mozaic in Seneca Falls, NY, is seeking a Planning Coordinator - Day Services/Community Habilitation to help people live meaningful, connected lives in their communities. You will coordinate services, advocate for individuals, and maintain documentation while promoting independence.

The role requires a High School diploma with at least one year of experience with developmental disabilities, strong communication and organizational skills, and a valid NYS driver's license.

Mozaic is looking for a Planning Coordinator - Day Services/Community Habilitation in Seneca County to join our growing team! This is a great opportunity for someone who is organized, compassionate, and passionate about helping people live meaningful, connected lives in their communities.

What You'll Do:

As a Planning Coordinator, you'll help ensure the people we support receive high-quality, person-centered services that reflect their goals, choices, and Life Plans. You'll advocate for the people we support, coordinate services and safeguards, maintain accurate documentation, and help oversee program and personal funds according to established procedures. You'll also build positive relationships with families, service providers, local businesses, and community partners while promoting greater community involvement and independence.

We're looking for someone who is dependable, organized, and comfortable balancing multiple priorities. Strong communication, teamwork, problem-solving, and time-management skills are important, along with the ability to work independently and handle confidential information professionally. Computer proficiency and a valid NYS driver's license are required, along with the ability to operate agency vehicles, including wheelchair-accessible vans, as needed.

Schedule:

Monday-Friday 8 am - 4:00 pm, with some flexibility

37.5 hours per week

Salary Range:

$19.53- $24.63 hr.

Planning Coordinator Qualifications:
