Mozaic is seeking a Planning Coordinator - Day Services/Community Habilitation in Seneca Falls to join our growing team. This role focuses on coordinating person-centered planning, safeguarding, and service delivery in the community setting, helping individuals live meaningful, connected lives.

You'll advocate for people supported, manage documentation, and collaborate with families, providers, and local partners. A valid NY driver's license and ability to operate agency vehicles are required.

Qualifications

• High School diploma or equivalent.
• 1 year of experience working with people with developmental disabilities.
• Valid NYS drivers license must be held.

Mozaic is looking for a Planning Coordinator - Day Services/Community Habilitation in Seneca County to join our growing team! This is a great opportunity for someone who is organized, compassionate, and passionate about helping people live meaningful, connected lives in their communities.

What You'll Do:

As a Planning Coordinator, you'll help ensure the people we support receive high-quality, person-centered services that reflect their goals, choices, and Life Plans. You'll advocate for the people we support, coordinate services and safeguards, maintain accurate documentation, and help oversee program and personal funds according to established procedures. You'll also build positive relationships with families, service providers, local businesses, and community partners while promoting greater community involvement and independence.

We're looking for someone who is dependable, organized, and comfortable balancing multiple priorities. Strong communication, teamwork, problem-solving, and time-management skills are important, along with the ability to work independently and handle confidential information professionally. Computer proficiency and a valid NYS driver's license are required, along with the ability to operate agency vehicles, including wheelchair-accessible vans, as needed.

Schedule:

Monday-Friday 8 am - 4:00 pm, with some flexibility

37.5 hours per week

Salary Range:

$19.53- $24.63 hr.

Planning Coordinator Qualifications:

High School diploma or equivalent required and 1 year of experience working with people with developmental disabilities.
