Mozaic in Seneca Falls, NY, is seeking a Planning Coordinator - Day Services/Community Habilitation to help people live meaningful, connected lives in their communities. You will coordinate services, advocate for individuals, and maintain documentation while promoting independence.

The role requires a High School diploma with at least one year of experience with developmental disabilities, strong communication and organizational skills, and a valid NYS driver's license.

Formación

• High School diploma or equivalent required and 1 year of experience working with people with developmental disabilities.
• Strong communication, teamwork, problem-solving, and time-management skills are important.
• Valid NYS driver's license and ability to operate agency vehicles as needed.

Responsabilidades

• Coordinate services and safeguards to reflect people’s goals and Life Plans.
• Advocate for people supported and maintain accurate documentation.
• Build relationships with families, service providers, and community partners.
• Ensure high-quality, person-centered services.
• Oversee program and personal funds according to established procedures.
