Job summary

Alpha Home Health and Hospice in New Jersey seeks a licensed Physical Therapist to provide one-on-one therapy to older adults in private residences and senior living communities. You will assess, treat, and monitor mobility, balance, strength, and functional independence, tailoring plans to each patient's goals while documenting progress in our EMR system.

Qualifications

• Licensed PT in New Jersey with valid license.
• Degree from an accredited PT program (DPT preferred).
• Basic computer and EMR skills required.
• Ability to drive to care sites and meet physical demands.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes and senior living communities.
• Deliver therapy to improve mobility, independence, and quality of life.
• Document patient progress in EMR and coordinate with an interdisciplinary team.
• Participate in telehealth/virtual care when appropriate.
