Qualifications

• MD or DO from an accredited medical school.
• Board Certified (BC) Internal Medicine, Family Medicine, or Geriatrics.
• Valid, unrestricted physician license in the state of operation.
• Must be opted-in with Medicare.
• Successful completion of an accredited residency program.
• Skilled nursing or long-term care experience strongly preferred.
• Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.

Responsibilities

• See roughly 16–25 patients a day, mixing long-term chronic care and acute issues.
• Manage wellness visits and H&Ps, coordinate with specialists and facility staff.
• Review medications and orders, respond to acute changes, and join care conferences.

About the Role

We're looking for an experienced, autonomous Physicianto join our clinical team Part Time at least 3 days per week and Full time, providing primary and episodic care toresidents in skilled nursing facilities. This is an independent role builtaround continuity you'll manage your own panel and build long-termrelationships with patients and their families.

Your Day-to-Day

You'll see roughly 16–25 patients a day a mix of long-termchronic care and acute issues.
