This independent role centers on continuity, allowing you to manage your own panel and build lasting relationships with patients and families. You'll round 16–25 patients daily, with 6-hour morning shifts, chart from home, and benefit from productivity-based compensation plus a long-term

Qualifications

• MD or DO degree from an accredited medical school.
• Board Certified in Internal Medicine, Family Medicine, or Geriatrics.

About the Role

Your Day-to-Day

You'll see roughly 16–25 patients a day a mix of long-termchronic care and acute issues. That means annual wellness visits and H&Ps,managing chronic and co-morbid conditions, responding to acute changes,reviewing meds and orders, and coordinating with collaborating specialists andfacility staff to keep care seamless. You'll also stay in touch with familiesand join care conferences where it makes sense.

Qualifications

• MD or DO from an accreditedmedical school.
• Board Certified (BC)requiredInternal Medicine, Family Medicine, or Geriatrics (or equivalent).
• Valid, unrestrictedphysician license in the state of operation.
• Successful completion ofan accredited residency program.
• Skilled nursing orlong-term care experience strongly preferred.
• Strong independentclinical judgment and the ability to move quickly in a fast-paced setting.
