Responsibilities

• Serve as the primary point of contact between the organization and healthcare providers across MI
• Driving referrals and admissions for a home care agency
• Building relationships throughout the healthcare space (hospital systems, SNFs, ALFs, discharge planners, case managers, social workers)
• Developing and maintaining referral pipelines
• Travel across Detroit counties to meet with key healthcare providers
• Conduct research to identify new providers in the region and explore opportunities for collaboration.
