Job summary

Qualifications
- Active licensure in Kentucky required.
- Current DEA license required upon start date.
- Current controlled substance license required upon start date.
- NCCPA certification required.
- Desire to work in Physiatry/PM&R.
- Orthopedic or Physiatry experience is a plus.
- SNF experience is a plus.
Responsibilities
- Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for SNF patients.
- Conduct thorough patient assessments and determine rehabilitation goals.
- Monitor progress and adjust treatment plans to optimize outcomes.
- Participate in multidisciplinary team meetings and educational activities.
- Maintain accurate documentation in EMR.
- Educate patients and families about rehabilitation and post-discharge care.
