Job summary

The APN brings patient-centered care, counseling, and coordination across care teams, including inpatient or procedural settings as needed. Strong licensing and NCCPA certification are required.

Responsibilities

• Assesses the physical and mental condition of patients by performing comprehensive or problem-focused exams and gathering histories.
• Orders, collects, interprets, and evaluates lab and diagnostic tests to assess patient needs.
• Prescribes or recommends medical therapies and performs procedures per approved protocols.
• Formulates and implements comprehensive treatment plans with physicians and team members.
• Counsels patients and families about medical processes, medications, nutrition, and health promotion.
• Makes timely referrals and maintains accurate treatment records.
