Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) is seeking a Nurse Practitioner or Physician Assistant to deliver primary and post-acute geriatric care in a Chicago-area Skilled Nursing Facility.

This full-time role offers a Monday–Friday schedule with no call, no nights, and no weekends, plus 4 weeks PTO, 1 week CME, and comprehensive benefits in a collaborative team environment. You will perform provider-level direct patient care and coordinate care with a multidisciplinary team.

Qualifications

• Must hold NP or PA licensure in Illinois.
• Geriatric care experience preferred.

Responsibilities

• Deliver provider-level direct patient care to geriatric patients within a Skilled Nursing Facility.
