Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) is seeking a Nurse Practitioner or Physician Assistant to deliver primary and post-acute care to geriatric patients in a Linwood, FL skilled nursing facility.

This full-time role emphasizes provider-level care with a Monday–Friday schedule and no on-call requirements. You will work with a collaborative, team-based environment to administer comprehensive medical care, coordinate treatment plans, and ensure high-quality post-acute services for

Qualifications

• Licensed NP or PA with credentialing to practice primary and geriatric care.
• Experience delivering direct care in a skilled nursing or long-term care setting preferred.
• Strong rounding, evaluation, and care coordination abilities.

Responsibilities

• Provide provider-level direct patient care to geriatric residents.
• Develop and implement treatment plans in collaboration with the facility team.
• Document care in EMR and coordinate with nursing and therapy staff.
