Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) is recruiting for a Skilled Nursing Facility in Linwood, NJ to hire a full-time Nurse Practitioner or Physician Assistant to deliver primary and post-acute geriatric care.

The role offers a Monday–Friday schedule with no call, no nights, and no weekends, plus 4 weeks PTO, 1 week CME, and full benefits. You will provide provider-level direct patient care within the facility and collaborate with the care team.

Qualifications

• NJ license required or eligible
• Geriatric primary and post-acute care experience preferred
• Facility-based care in a Skilled Nursing Facility setting

Responsibilities

• Deliver primary and post-acute geriatric care in a SNF
• Perform comprehensive assessments and care planning
• Coordinate with interdisciplinary team for patient progress
• Document care in the EMR and ensure regulatory compliance
