Northeast Healthcare Recruitment, Inc. (NEHCR) is assisting a Skilled Nursing Facility in Naples, FL in hiring a Nurse Practitioner or Physician Assistant for full-time, weekday primary and post-acute geriatric care.

The position offers a Monday-Friday schedule with no call, no nights, and no weekends, plus full benefits, 4 weeks PTO and 1 CME week in a collaborative team-based environment. The primary role is provider-level direct patient care within the SNF.

Qualifications

• Experience in geriatric primary and post-acute care.
• Ability to deliver provider-level direct patient care in SNF settings.

Responsibilities

• Provide provider-level direct patient care to geriatric patients in skilled nursing facility.
• Collaborate with interdisciplinary teams to coordinate patient care.
• Support transition of care and post-acute interventions.
