Job summary

Top Echelon Software, LLC, in the Greater Sacramento area, CA, seeks a full-time Advanced Practice Clinician (NP/PA) to provide comprehensive primary care in a community health center setting. This role offers opportunity to serve diverse populations while collaborating with physicians and multidisciplinary teams.

The ideal candidate holds a CA PA license, NCCPA certification, DEA, and BLS, with 1–2 years of clinical experience, particularly in community health.

Qualifications

• Active California Physician Assistant license.
• NCCPA board certification.
• Current DEA certification.
• Current BLS certification.
• 1–2 years of clinical experience preferred, especially in community health or underserved populations.
• Strong commitment to serving low-income, homeless, or medically underserved communities.
• Bilingual skills in English and Russian, Farsi, Pashto/Pushto, or Spanish preferred.

Responsibilities

• Perform comprehensive patient evaluations, including history and physical exams.
• Diagnose and manage acute and chronic medical conditions.
• Order and interpret laboratory tests, imaging, and other diagnostics.
• Prescribe medications and therapies within scope.
• Develop and implement treatment plans with Medical Director.
• Monitor progress and adjust care plans as needed.
• Initiate emergency procedures when indicated.
• Provide preventive care, health education, and counseling.
• Maintain accurate documentation per standards.
• Collaborate with physicians and care teams for coordinated care.
