Clear Concepts, Inc. is seeking a full-time Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to deliver comprehensive primary care in a community health setting near Sacramento.

The role emphasizes serving diverse, underserved populations and working with a multidisciplinary team of physicians and staff. Responsibilities include patient evaluations, diagnosing and managing conditions, lab testing, prescribing within scope, care plan development, and preventive health

Qualifications

• Active California PA license.
• NCCPA board certification.
• Current DEA registration.
• Current BLS certification.
• 1–2 years of clinical experience in community health preferred.
• Commitment to serving underserved communities.
• Bilingual in English and Russian, Farsi, Pashto/Pushto, or Spanish preferred.

Responsabilités

• Perform comprehensive patient evaluations, including history and physical examinations.
• Diagnose and manage acute and chronic medical conditions.
• Order and interpret laboratory tests, imaging, and other diagnostics.
• Prescribe medications and therapeutic interventions within scope of practice.
• Develop and implement treatment plans in collaboration with the Medical Director.
• Monitor patient progress and modify care plans as appropriate.
• Initiate emergency procedures when clinically indicated.
• Provide preventive care, health education, and counseling services.
• Maintain accurate and timely documentation in accordance with agency and regulatory standards.
• Collaborate with physicians and other healthcare professionals to ensure coordinated, high-quality care.
