Top Echelon Software LLC seeks a full-time Advanced Practice Clinician (Physician Assistant or Nurse Practitioner) to deliver comprehensive primary care in a community health center. Located in the Greater Sacramento area, this role offers responsibility for diverse populations and collaborative teamwork across physicians and care teams.
Requirements include an active California PA license, NCCPA certification, DEA and BLS, with 1–2 years in community health preferred.
Qualificações
- Active California Physician Assistant license.
- NCCPA board certification.
- Current DEA certification.
- Current BLS certification.
Responsabilidades
- Perform comprehensive patient evaluations, including history and physical examinations.
- Diagnose and manage acute and chronic medical conditions.
- Order and interpret laboratory tests, imaging, and other diagnostics.
- Prescribe medications and interventions within scope of practice.
- Develop and implement treatment plans in collaboration with Medical Director.
- Monitor patient progress and modify care plans accordingly.
- Initiate emergency procedures when clinically indicated.
- Provide preventive care, health education, and counseling services.
- Maintain accurate documentation per agency standards.
- Collaborate with physicians to ensure coordinated, high-quality care.
