Premise Health is seeking a full-time Physician Assistant to provide primary care to employees and dependents at our Health Center in Lafayette, IN. The role supports preventative care, annual wellness exams, and women's health services within a collaborative team.

Uncompromising patient-centered care with flexible 4-10 or 5-8 shifts, and clinic hours from 7:00 AM to 7:00 PM. Opportunity for professional growth, comprehensive benefits, and involvement in long-term patient health management.

Competenze

• Current NCCPA national certification required.
• Degree from ARC-PA accredited program.
• Federal DEA license (matching state of practice).
• State license to practice in Indiana.

• Develops patient care plans, records progress, and ensures continuity of care.

As a Full Time Physician Assistant, you'll provide care to client employees and their dependents in our Health Center located in Lafayette, IN.

Position Highlights

• Full-time position (40 hours per week)
• Monday-Friday schedule with flexible options of 4 10-hour shifts or 5 8-hour shifts
• Clinic hours: 7:00 AM - 7:00 PM with a rotating schedule that includes two early shifts and two late shifts
• A patient-centered environment where providers have the time they need to deliver high-quality, personalized care-no rushed visits
• Strong focus on preventative care and long-term patient health management
• Patient visits commonly include annual wellness exams, preventative health screenings, and women's health services
• Patient population ages 10 and up
• Primary care experience preferred
• Collaborative care team with strong clinical support

What You'll Bring

• Maintains state licenses/certifications to practice
• Maintains Board of Pharmacy Prescribing License (if applicable)
• Current NCCPA national certification required
• Degree from ARC-PA accredited program
• Federal DEA license (matching each state of practice as applicable)
• State Narcotic/Controlled Substance license if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico) (New York ROPES registration)
• Current certification with hands‑on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services
• At least three years of clinical experience as a Physician Assistant in an ambulatory care, occupational health, family practice or emergency department setting
