LionStone Care in Youngstown, OH is seeking a Per Diem Physical Therapist Assistant to support therapy services at Park Vista - Therapy. The role offers an hourly wage of $32.00 - $35.00 and requires an Associate degree with active Ohio licensure.

Responsibilities include assisting the Physical Therapist, coordinating Telehealth evaluations, documenting care, and ensuring accurate billing and compliance with standards. Per diem position with full on-site duties.

Qualifications

• Must possess an Associate degree from an accredited program conferring the title of Physical Therapist Assistant.
• Must possess a current, unencumbered Ohio license to practice as a Physical Therapist Assistant.

Responsibilities

• Assist the Physical Therapist in planning, development, organization, evaluation and directing of Physical therapy activities.
• Facilitate Telehealth services to ensure evaluations, recertifications, progress notes and discharges are completed timely.
• Assist in care conferences, discharge planning and resident assessments as necessary.
• Provide individualized physical therapy services to residents according to the Plan of Care.
• Ensure that notes are informative and reflective of care provided and resident responses.
