Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Johnstown, PA. The role supports therapy services under the supervision of a Physical Therapist and participates in establishing treatment plans and goals.

Applicants should hold an Associate's degree in physical therapy and an active PTA license. Full-time or PRN options are available with benefits based on status.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to continuing education and professional development.

Responsibilities

• Provide PT services to residents under supervision of a Physical Therapist.
• Conduct screenings to determine need for intervention or treatment.
• Assist in evaluations and treatment planning under PT guidance.
• Develop and modify resident treatment schedules with the PT and care team.
• Educate residents, family, and staff on therapy modalities and equipment.
• Document compliant and medically necessary CMS-based documentation in NetHealth EMR.
