Responsibilities

• Educates patients, family, caregivers, students, and other health care providers using relevant teaching methods.
• Follows the plan of care and adjusts treatment based on patient response and documented communication with evaluating therapist.
• Produces documentation per Clinical Documentation Policy.
• Completes annual education to comply with program requirements.
